How
Do I Enroll in Healthy Start?
As soon
as you think you may be pregnant, schedule an appointment with your
health care provider. Upon selection of your prenatal
provider, ask for the Healthy Start Prenatal Screen. This is a
simple confidential one-page questionnaire that is completed by you
and your health care provider. The answer to these questions
assists your health care provider to provide you with the best
possible care.
You may also call the
Healthy Start office at 772-770-5151, ext. 4 for more information or
to request services.
