Healthy Start Mission Statement
The
Healthy Start Care Coordination Team is dedicated to improve the
health outcome of pregnant women and infants through education,
empowerment, and coordination of community resources.
About Healthy Start
Healthy
Start is a voluntary program that provides moms and newborns with
the resources, support, and help they need to develop a healthy
family outcome. Healthy Start is for:

How
do I enroll in Healthy Start?
As soon
as you think you may be pregnant, schedule a visit with your health
care provider. During the initial visit, ask for the Healthy Start
Prenatal Risk Screen. This is a simple, one-page questionnaire that
is completed by you and your health care provider. The answers to
these questions are kept confidential and are used only to provide
the best possible care for you and your baby.
You may also self-refer by calling the Healthy Start office
at (772) 770-5151, select option 2.
