HOW TO CONTACT US
Indian River County Health Department's Office of
Vital Statistics has a record of births and deaths that occurred in
Indian River County. Certified copies of birth and death
records (certificates) may be obtained from our office located at
1900 27th Street, Vero Beach, FL 32960. Our phone number is (772)
794-7460. Office hours are Monday through Friday, 8:00 am to 4:30
pm. Identification will be required.
BIRTH CERTIFICATES
A
certified birth certificate is an important legal document to be
used for proof of age, social security registration, tax
information, entering school, obtaining a passport or driver's
license, and has many other uses. We have
birth certificates for individuals born in Indian River County from 1933 to
present. In addition, we have the ability to print certified
copies of birth certificates from any county in the state of Florida
for all years with the exception of births that occurred prior
to 1930.
A
written request is required for all birth certificates. They can be
obtained with minimal waiting time directly from our office or you
can mail in your request by following the guidelines below:
Written request must include name as listed on the birth
certificate

Father's name and mother's maiden name

Original signature of requester

Check
or money order for the cost of the certificate (see Fees below)
payable to "IRCHD" or "Indian River County Health Department."

copy of government issued ID (i.e. driver's license, passport,
military, etc.)

Address to mail the certificate to and a telephone number.
In
order to comply with statutory laws, we can only release birth
records to a parent listed on the certificate, to a legal guardian
following proof of guardianship or to the individual listed if they
are 18 years or older.
Birth Certificate Forms and Additional Information
DEATH CERTIFICATES
Death certificates are available from 1944 to present. All death
records, which exclude the cause of death, are public record
.
In
order to comply with statutory laws, death certificates with the
cause of death listed can only be released to immediate relatives,
the executor of the estate, or to an individual listed on an
insurance document. A
written request is required for all death certificates. They can be
obtained with minimal waiting time directly from our office or you
can mail in your written request by following the guidelines below:

Written request must include the name of the decedent

Date
of death

Whether you want the certificate with or without cause of death
(limits apply for release with cause of death)

Name
and relationship of person requesting the certificate including
original signature.

Check
or money order for the cost of the certificate(s) - See FEES
below - payable to IRCHD or Indian River County Health
Department.

Photo
copy of government issue ID (i.e.
driver's license, passport, military, etc.) if you are
requesting a copy with cause of death

Address to mail the certificate and phone number. It is
recommended that you include a self-addressed stamped envelope for
faster processing.

*FEES
BIRTH
CERTIFICATES:
A fee
of $12.00* is charged for the first copy of a birth certificate, and
additional copies of the same certificate may be purchased
for $10.00*, if requested at the same time.
DEATH
CERTIFICATES:
A fee
of $12.00* per certified copy. There is a $3.00* charge per year for
death searches if the year of death is unknown.
Checks are accepted and should be made
payable to IRCHD or Indian River
County Health Department.
